|
At Jefferson Lab a Confined Space Entry (CSE) Permit is considered
to be a Work
Control Document and thus a Standard
Protecting Measure. Only when all hazard
issues associated with the PRCS and the work activity are acknowledged and
mitigated, in accordance with this program, can the area’s associated Risk Code be
considered < 2. At a
minimum, all work requiring entry into a PRCS is discussed with the entry
supervisor prior to commencement. A formal Task Hazard
Analysis and Operational
Safety Procedure is required for work considered to be Risk Code >2 in
accordance with ES&H
Manual Chapter 3210 Work Planning, Control, and Authorization Process. |
1.0
Purpose
An approved Confined Space Entry (CSE) Permit is required prior to entry into a Jefferson Lab Permit Required Confined Space (PRCS). This appendix provides the process steps for completing the CSE Permit.
2.0
Scope
A Jefferson Lab CSE Permit documents required mitigation for a specific PRCS and is valid only for the time period designated on the permit.
Jefferson Lab’s CSE Permit cannot be used to authorize entering into sewer manholes, wastewater pump pits, or similar areas. For information on entry into these spaces contact the Confined Space Coordinator.
This appendix is written in coordination with ES&H Manual Chapter 6160 Permit-Required Confined Space Entry Program.
3.0
Responsibilities
NOTE: Management authority may be delegated at the
discretion of the responsible manager.
3.1
Qualified
Attendant
· Maintain appropriate training (SAF907 Confined Space Entry, or equivalent as approved by the Confined Space Coordinator).
· Read and abide by the requirements listed on the CSE Permit. (Suspend work (see ES&H Manual Chapter 3330 Appendix T1 Suspend Work for Safety Procedure) if conditions are altered.)
· Ensure that work remains consistent with the terms of the permit. (Order an evacuation if conditions are altered.)
· Prevent unauthorized entry into the PRCS.
3.2
Authorized
Entrant
· Maintain appropriate training (SAF907 Confined Space Entry, or equivalent as approved by the Confined Space Coordinator).
· Read and abide by the requirements listed on the CSE Permit. (Suspend work (see ES&H Manual Chapter 3330 Appendix T1 Suspend Work for Safety Procedure) if conditions are altered.)
3.3 Entry Supervisor/Subcontracting Officer’s Technical Representative (SOTR)/ Sponsor
· Maintain appropriate training (SAF907 Confined Space Entry, or equivalent as approved by the Confined Space Coordinator).
· Complete the CSE Permit per this appendix. Request assistance from the Confined Space Coordinator as required.
· Assign only Qualified Attendants and Authorized Entrants to perform work within a PRCS.
· Ensure Qualified Attendant(s) and Authorized Entrant(s) read and understand the requirements listed on the CSE Permit.
· Submit all confined space entry documentation to Confined Space Coordinator after entry is complete.
3.4 Confined Space Coordinator
· Maintain copies of CSE Permits.

Confined
Space Entry Permit ![]()
The Entry Supervisor completes the CSE Permit prior to authorizing entry into any recognized PRCS.
Step 1 Introduction:
· Space Designation, Nearest Permanent Building/Location, and Type of Space: Using ES&H Manual Chapter 6160 Appendix T1 Permit-Required Confined Locations locate the PRCS to be entered, find its associated number on the tables provided, and fill in the blanks on the form with the requested information.
· Start Date/Time: When work will commence.
· Estimated Duration: Approximate number of hours to do job (No more than eight hours.)
· Purpose of Entry: State tasks to be performed.
Step 2 Hazard Control Measures:
Denote with a Ö all that apply and describe as requested. Consult system experts or Subject Matter Experts when assistance is needed.
Step 3 Equipment
· Required
o Emergency Communications –always required.
o Retrieval Gear (lifeline and body harness) - Required unless it constitutes a greater hazard, or hinders rescue operations. Some typical contra-indications:
§ The PRCS has objects that could snag the lifeline or harness.
§ The lifeline poses a significant trip hazard.
§ The route taken by the entrant makes retrieval impossible without injuring the entrant.
§ By prior agreement between Emergency Manager, Confined Space Coordinator, and Newport News Fire Department.
o A retrieval hoist is required for entry into below-grade spaces.
· Based on Actual Conditions
o Lighting (vapor-tight)
o GFCI for electrical equipment
o Portable generator
o Pump
o Batteries
o Other - Describe
Step 5 Role Assignment
Designate the individuals assigned to the following roles. Discuss the work assignments including frequency and responsibility of atmospheric testing.
· Authorized Entrant(s)
· Qualified Attendant(s)
Step 4 Authorizing Signature
· Entry Supervisor – Individual who:
o Completes the form
o Assigns work responsibilities to Entrant(s) and Attendant(s)
4.2 Atmospheric Testing
The Entry Supervisor, Qualified Attendant, or Authorized Entrant may perform Atmospheric Testing as designated by the Entry Supervisor.
· Instrument’s Make, Model, and Serial No. – Obtain appropriate instrument from Industrial Hygiene. (See ES&H Manual Chapter 6160 Appendix T4 Atmospheric Testing and Ventilation Monitoring of Permit-Required Confined Spaces).
· Calibration Expiration Date – Instrument’s calibration expiration date. (The last date an instrument may be used.)
· Person Performing Test (Initials) - Initial to indicate that test was performed at indicated interval.
· Testing
o Determine testing schedule (minimally hourly).
o Perform the first test from outside the PRCS. (If this cannot be accomplished, contact the Confined Space Coordinator.)
o Note the findings on the first column of the permit. Subsequent testing may be performed from within a PRCS if conditions remain acceptable.

5.0
Documentation
When the entry is completed:
· Permit Closeout– the Entry Supervisors signs the permit with the date and time of completion.
· Submit the Closed Permit – To the Confined Space Coordinator for filing.
