All Test Lab Workers,

At the beginning of last week, a potential for oxygen deficiency
occurred in the Test Lab Rom 146B ( QA Room).  A hand held nitrogen gun
fitting came loose sometime over the weekend.  This resulted in a
constant flow of approximately 8 cfm of nitrogen into the room.  The
source of the flow is the 3,000 gallon liquid nitrogen dewar.  The room
has no engineered fresh air intake, so the room had the potential to
become inerted.  This presented a potentially serious hazard for our
staff.  The source of nitrogen to this room has been eliminated, and
other areas across the test lab have engineered controls put in place.
However, corrective actions are being implemented to ensure that all ODH
controls recommended by the Cryogenic Coordinator's Oxygen Deficiency
Hazard Analysis for the Test Lab are present and are functioning ,
including administrative controls such as proper posting of ODH areas.
The first corrective action that you should be aware of is that,
effective immediately, the Test Lab High Bay work areas and all rooms
including offices that open to the High Bay are treated as ODH 0.  The
perimeter is being posted as ODH 0 in a manner similar to the existing
GERT posting.
Be aware that there are  work areas that are already posted with the
appropriate ODH status, for example the Cryomodule Test Cave.
The requirement for work in an ODH 0 area is ODH training found at
http://www.jlab.org/div_dept/train/lms/ODH_Test/TPO-ODH_Exam.tbk.  This
training is required every 2 years.

To bring visitors into an ODH 0 area, you have the option to have them
take the training, or to provide a hazard awareness briefing.  The
official briefing material is attached.
