New Hall D Task List




Hello all,

The accelerator  has been using a system for quite a few years that captures all the work done in the accelerator and applicable areas. It is called ATList. It is web based and serves many functions. It helps in planning work safely, contains a task hazard analysis worksheet, informs the applicable people, helps coordinate the flow of work in the area, documents the work and provides a place for feedback. Halls A, B and C have recently adopted this and Hall D will use it also. This is usually reserved for physical tasks. The accelerator computer folks have kindly "cloned" the system so we can use it in Hall D. The system is called HDlist and can be accessed through clicking on the link below (sample task). I have put in a sample task. You can start looking at this and making comments immediately. The fields will change as we discover what is best for our hall. Use your regular JLAB login to access. If you are interested in learning more, you can contact me.This will eventually be tied into the Hall D elog when it is available.

Hazard Identification Worksheet
NO Are there MSDS requirements for the materials being used with which you are unfamilar. or does the task require Personal Protective Equipment (PPE)?
NO Will you create dust, welding arcs, heat, excessive noise, ionizing/non-ionizing radiation, or chemical mixtures during the tasks?
NO Are there any fire or explosive hazards associated with the task or likely to develop because of the task?
YES Could the task create headaches, breathing problems, or dizziness from odors, etc.?

Argon/Co2 while connecting gas lines if not done in a well ventilated area
YES Is the task performed where limited entry, egress, or poor ventilation exists?

Person must actually get in BCAL which is a confined area. Need to evaluate area and egress and work accordingly
YES Does the task require compressed, liquefied, or noxious gases?

Compressed gases to be stored outside hall
NO Does the task require work in areas or with materials subject to temperature extremes?
YES Does the task involve the use of fork trucks, hoists, or cranes?

Need to crane the FDC assy onto platform. Need to have lift plan and follow ESH&Q manual chapter 6140. Crane operators must be trained and current.
NO Does the task involve the use of powered hand tools?
YES Does the work involve the risk of electrical shock or other forms of hazardous energy?

Ensure all electronics are powered off before connecting
YES Does the task involve working above or below floor level?

Work on platform. Rails are installs.
YES Does the task involve lifting, pulling, pushing, or carrying heavy objects, or repetitive motion?

Use proper techniques when pulling FDC into BCAL. Evaluate the use of an electric winch.
YES Does the task involve work with pressurized or vacuum vessels?

FDC is slightly pressurerized but does not pose an explosion hazard since pressure cannt get hignh enough
NO Does the task require any permits?
YES Does the task require specialized training?

Each portion of the task will be performed by trained technicians (crane, electronics, gas system)
YES Will waste products require special handling or disposal requirements?

Exhaust gas will be vented to the outside
NO Any other hazards we may have overlooked with this list?

Description of Task

This is only a sample task! Bring FDC from EEL builing to Hall D.
Install the FDC into the BCAL. This will involve craning the FDC onto
the upstream platform, installing cables, sliding it into the BCAL,
hooking up gas lines and cooling lines.
