revisions to the Hall D / accelerator interface document

Please find attached my revisions to the interface document that Tim 
sent around for review.  I would also like to hear any comments coming 
back from the machine group.  The document is in pdf format, with 
sticky-notes giving my suggested changes.  I have provided information 
backing up my changes in the notes.  You may need to scroll down inside 
the notes boxes to see the full text.
